Aetna Wellness Counseling

Aetna Wellness Counseling offers personalized behavior change support to members whose health is at high risk, including resources and tools to help them lower their risk factors — and improve their overall health.

If you have a Health Risk Assessment score that places you in the high or very high risk category, and you have voluntarily agreed to be contacted, you will be assigned to one of Aetna’s Wellness Counselors (registered nurses, dieticians and other experienced health educators).

Wellness Counselors are trained to help you succeed at making healthy lifestyle changes, and help you develop, implement and follow a personalized health Action Plan.

During the initial call, the Aetna Wellness Counselors will:

  • Review your Health Risk Assessment
  • Determine how well you know your specific risk factors
  • Help you understand your personalized Action Plan
  • Identify opportunities for referral to other Aetna programs (e.g., Disease Management, Women’s Health, etc.)
  • Review available health information resources (e.g., Informed Health® Line, preventive health schedules, Healthwise® Knowledgebase, etc.)
  • Establish a personalized educational and follow-up plan.

Ongoing program activities are determined based your readiness to change indicators on the Simple Steps To a Healthier Life® Health Risk Assessment, as well as on individual needs and preferences.

Additional Support with Making Positive Health Changes
Members may also choose to participate in the Simple Steps To A Healthier Life Healthy Living Programs. These innovative, online programs are incorporated into members’ personalized Action Plans based on their Health Risk Assessment responses, and offer a step-by-step approach to making positive health changes.
